AB A 21-year-old Caucasian male presented with acute and severe necrotising
oral infection. He had been diagnosed HIV positive two months
previously. His CD4 count was 726, and he was not taking any medication
related to his HIV infection. He was a homosexual IV drug user.
Immediate Antibiotic therapy was instituted with good relief of symptoms
within 24 hours. Subsequent dental treatment, including sequestrectomy,
tooth extraction and periodontal treatment resulted in complete
resolution of symptoms. He is currently well. Severe oral disease may be
the first clinical manifestation of HIV infection. Early diagnosis,
aggressive oral treatment and antibiotics resulted in resolution of
symptoms. Oral health care screening and dental advice at the time of
HIV diagnosis may have prevented these conditions from developing.
